Choosing the Right Form
There are 3 different forms to choose from when you need a transcript or credit letter. Please read the following choices to determine which form you need.
CE Participants
This form is to be used by individuals who need a copy of their Continuing Education Confirmation Payment, Unofficial Transcript or Credit Letter. The cost is $25. Confirmation Payment and Credit Letters are free if requested more than 90 days following the completion of the course.
Currently Enrolled Students
This form is to be used by currently enrolled students who need a copy of their academic transcript, immunization records and/or NBEO scores. There is no charge for this service.
Alumni and Former Students
This form is to be used by alumni and former students of the College who need a copy of their academic transcript. The cost per copy is $20 for this service.
